This Home Health Physical Therapist opportunity is based in
Eufaula, Alabama, a picturesque lakeside town along the
Chattahoochee River. Eufaula is known for its beautiful views of
Lake Eufaula, also called the Bass Capital of the World, offering
plentiful opportunities for fishing, boating, and outdoor
recreation. The historic district showcases charming antebellum
homes, tree-lined streets, and local shops and restaurants that
create a warm, welcoming community atmosphere. Residents enjoy a
relaxed, small-town feel with easy day-trip access to larger cities
such as Dothan, Montgomery, and Columbus, Georgia, giving you both
tranquility and convenience.In this role, you will provide
one-on-one physical therapy services to patients in their homes
throughout Eufaula and the surrounding area. You will conduct
comprehensive assessments, develop individualized plans of care,
and implement treatment interventions focused on improving
mobility, safety, strength, and independence. Your caseload will
primarily include adult and geriatric patients recovering from
surgeries, orthopedic injuries, neurologic events such as stroke,
and those managing chronic conditions that affect functional
mobility.Typical responsibilities include completing in-home
evaluations, designing and progressing therapeutic exercise
programs, providing gait and balance training, recommending durable
medical equipment, and coaching patients and caregivers on home
safety and fall prevention. You will collaborate closely with an
interdisciplinary team that may include nursing, occupational
therapy, speech therapy, social work, and home health aides to
ensure coordinated, patient-centered care. Documentation is
completed using an electronic medical record system, with an
emphasis on timely, accurate visit notes and outcome tracking.A
typical day involves traveling between patients homes within an
assigned territory around Eufaula, performing scheduled visits,
communicating with the clinical team, and adjusting care plans as
needed. Visit volumes are structured to support thorough,
high-quality care and allow adequate time for education and
hands-on treatment with each patient. You will have a high level of
autonomy in organizing your schedule while still being supported by
clinical leadership and office-based staff.Working in home health
