Primary Care Physician – multiple openings Job Overview: This role
involves independently delivering care to patients with both acute
and chronic conditions, particularly in the older adult
demographic. You will take full responsibility for patient care and
outcomes, seeking specialist consultations when necessary while
remaining actively involved in the patient's ongoing care.
Collaboration with hospitalists is essential whenever your patients
are hospitalized, regardless of their affiliation. Your
responsibilities will include assessment, diagnosis, treatment,
management, education, health promotion, care coordination, and
documentation for patients with complex health needs. For patients
unable to visit the office—whether in hospitals, skilled nursing
facilities, long-term care, or homebound—you will work closely with
the transitional care team and other available resources, including
case managers and acute care physicians. You may also lead weekly
Super Huddle (SH) and Transforming Care Meetings (TCM) and provide
support to the Center Manager or Center General Manager in their
absence, following guidance from the Market Chief Medical Officer.
You may be required to work at various locations within Duval
County. Qualifications: A Medical Degree or its foreign equivalent,
along with Board Certification or Eligibility (BC/BE) in Family
Medicine or Internal Medicine. A valid Florida State Medical
license and a Drug Enforcement Administration (DEA) license are
mandatory. Job Location: 3059 Edgewood Ave, Jacksonville, FL 32209.
Additional work may be required at other sites within Duval County.
SALARY RANGE: $214,700 - $306,714 EMPLOYEE BENEFITS We are
