The Physical Therapist will assess patients' conditions, develop individualized treatment plans, and provide hands-on therapy sessions to aid in rehabilitation and recovery. Responsibilities include monitoring patient progress, collaborating with other healthcare professionals, educating patients and families about therapeutic exercises, and ensuring a safe and effective care environment. Additionally, the Physical Therapist will deliver high-quality care in a home care setting as needed. Qualifications Expertise in Physical Therapy and Rehabilitation methods Proficiency in Patient Care and experience providing Home Care services Background in Medicine as it pertains to physical health and mobility Strong interpersonal and communication skills to interact effectively with patients and multidisciplinary teams Licensed to practice as a Physical Therapist in Florida CPR and First Aid certification (preferred) Proven ability to design and implement treatment plans for diverse needs Prior experience in a home healthcare or clinical setting is a plus
