to the community! Responsibilities About the Role During visits
that may last up to one hour, our providers assess and observe a
member’s current health status, medical history, medication
adherence, social circumstances, and other potential risks. This
approach provides invaluable insights into a member’s overall
health that are often missed during standard office visits. The
Matrix Comprehensive Health Assessment (CHA) enhances care quality
and allows us to address multiple care gaps in a single
appointment. Our providers are responsible for: Conducting
assessments for adults and geriatrics, including medical history,
diagnosis and treatment, health education, physician referrals,
case management referrals, follow-up, and thorough documentation in
line with our guidelines and protocols. Administering and ordering
necessary point-of-care and lab-based tests. Collaborating with
physicians, case managers, social workers, family members, key
caregivers, and other medical personnel as needed. Working
alongside Primary Care Physicians (PCPs) to provide patient
education and follow-up as required. Delivering services at the
member’s residence, which may include their homes or Skilled
Nursing Facilities. Benefits of Working with Us Consistency – While
assessments are conducted from Sunday to Saturday between 8 am and
7 pm, our full-time providers generally work Monday to Friday from
8:00 am to 5:30 pm. Competitive Pay – Enjoy an annual base salary
ranging from $95,500 - $158,400 , determined by market analysis and
experience. Incentive Program – Includes uncapped monthly
incentives and year-end bonuses. Comprehensive Benefits – Mileage

education opportunities. Additional Licensure – If business needs
arise, we will assist you in obtaining additional state licensure
and credentialing in neighboring states or others you may wish to
visit while conducting health assessments. Qualifications Skills &
Experience That Shine A Master’s Degree or equivalent experience
along with successful completion of NP or PA licensure. Current
NP/PA licensure in the state of practice, including prescription
authority or the ability to obtain it as necessary. Board
certification from NCCPA, AANP, AACN, or ANCC in Adult, Adult
Gerontology Primary Care, Adult Gerontology Acute Care, Family,
Emergency, or Acute Care specialty. Current BLS, ACLS, or CPR
certification. Preferred one year of NP or PA experience; however,
new graduates are encouraged to apply! Willingness to travel up to
15% of the time (including overnight travel) to various locations
as required. A valid state driver’s license and proof of adequate
automobile insurance for your state of residence. Strong computer
skills and familiarity with health/medical record software.
