Job Description Job Description Overview Comments The Lead
Oncology Pharmacist is responsible for the provision of
pharmaceutical care to oncology patients in the inpatient and
outpatient settings and serves as an expert consultant within the
specialty practice areas of oncology and supportive care/symptom
management. Providing pharmaceutical care within the scope of this
position includes but is not limited to supervising support staff,
participating in both clinical and dispensing activities related to
chemotherapy, drug utilization evaluations, formulary monitoring,
formulary monographs/proposals within the oncology practice area,
drug information requests, adverse drug reaction monitoring, and
education of colleagues, patients, and providers alike. This person
may provide pharmacy services to patients from the child,
adolescent, adult and geriatric age groups. Responsibilities
include (but are not limited to) overseeing educating, and
directing the pharmacist team in these areas and addressing work
flow issues and process improvement. Acts in a collaborative role
with the medical and hospital staff. Continuous involvement in
various interdisciplinary committees for protocol development and
implementation. Maintains responsibility for education of
pharmacists, pharmacy residents, and pharmacy students as it
relates to medical oncology and immunotherapy. This position's work
location is on site only. Responsibilities Leadership * Serves as a
resource for staff in caring for complex patients in the specialty
area. * Provides oversight and development of the team concept for
the designated clinical area. * Serves as a role model for
department staff and participates in special projects. * Serves as
a liaison with the medical and hospital staff in
providing/developing services, protocols, and procedures to meet
the needs of patients. * Collaborates with other pharmacy leaders
and the pharmacy team to achieve compliance with hospital and
department specific policies and procedures. * Educates medical,
nursing, and pharmacy staff as it relates to the specialty practice
area. * Demonstrates engagement and fosters engagement in others. *
Serves on designated intra- and inter-departmental committees to
improve patient care. Patient Care * Performs clinical services to
optimize drug therapy and documents clinical interventions. *
Provides medication therapy management using evidence-based
medicine, literature, guidelines, and protocols. * Uses knowledge
of pharmacokinetics and pharmacodynamics, therapeutic drug
monitoring, and patient-specific data to maximize medication
outcomes. * Demonstrates knowledge in the utilization of pharmacy
technologies such as electronic health records and clinical
decision support/data mining tools/systems. * Advises physicians
and other prescribers of formulary medications and suggests
alternative therapeutics regimens/options. * Provides accurate,
clinically relevant drug information to physicians, nurses and
allied health professionals. * Actively contributes to
multi-disciplinary decision making in the specialty practice area.
* Reviews, interprets, and verifies physician orders accurately;
screens for incompatibilities, interactions, duplications, and
contraindications. * Provides discharge medication patient
counseling (new and previous home medications, using verbal and
written communication) as needed. * Processes inpatient and
outpatient oncology/chemotherapy medications, supportive care, and
pre-medication orders accurately and effectively. * Identifies,
prevents, and investigates potential adverse drug reactions and
medication errors. * May be required to process and/or dispense
non-oncology related orders and medications depending on business
needs. Administrative * Develops programs, policies, and guidelines
that support evidence-based standards of practice within the
specialty practice area. * Assists in the development and
maintenance of a chemotherapy and immunotherapy formulary. *
Compiles and reports to the P&T committee data / metrics on
chemotherapy use when requested. * Promotes compliance with
requirements of the HRSA 340(b) program, as appropriate. *
Supervises supportive personnel, licensed and non-licensed, and
organizes/directs workflow. * Exercises applicable legal and
professional controls of controlled substances; documents waste
from returned/expired/unusable controlled substances properly. *
Performs drug utilization reviews with appropriate/accurate
documentation. * Reports adverse drug reactions, medication errors
and good catches. * Assists in maintaining basic drug supply from
vendors. * Pro-actively identifies opportunities for process
improvement. Compliance/Safety * Responsible for reporting any
safety-related incident in a timely fashion through the Midas/RDE
tool; attends all safety-related training programs; performs work
in a safe manner; monitors work environment for possible safety
issues and ensures others are also performing work in a safe
manner. * Stays current and complies with state and federal
regulations/statutes and company policies that impact the
employee's area of responsibility. * If required for the position,
ensures all certifications and/or licenses are up-to-date and valid
prior to expiration dates. * Completes all company mandatory
modules and required job-specific training in the specified time
frame. Qualifications Education Bachelor of Science in Pharmacy
from an accredited college of pharmacy - Required Doctor of
Pharmacy degree from an accredited college of pharmacy - Preferred
Certification & Licensures Arizona State Board of Pharmacy license
that is active and in good standing - Required at date of hire
Board of Pharmacy Specialties Certification in Oncology (BCOP) at
date of hire, or obtained within 2 years from date of hire -
Required BLS certification - Required ACLS certification - Required
ASHP Sterile Product Preparation Training and Certificate Program-
Preferred Experience PGY2 Residency in Oncology - Preferred CALL
REQUIRED : Must be able to physically show up Call required as a
subject matter expert; physical response may not be necessary
