Join as a traveling Physical Therapist where you will help patients
recover from injuries and manage chronic conditions while working
in home health across the Opp, AL area. Apply now and a recruiter

degree in Physical Therapy (Bachelor's, Master's, or Doctoral) from
an accredited program approved by the APTA. Active Alabama Physical
Therapy license required to start the assignment. Candidates with
multi-state licensure covering AL may be considered. Current
BLS/CPR certification from AHA or ARC required. Valid driver’s
license, vehicle insurance, and access to a dependable vehicle for
patient visits in the service area. Experience providing physical
therapy in home health or community-based settings preferred.
Ability to complete evaluations, develop plans of care, and
document per CMS and agency standards within required timeframes.
Willingness to participate in weekend rotation and be available on
observed holidays as needed. Schedule flexibility required. Ability
to supervise PTAs and home health aides as permitted by state
regulation and agency policy preferred. Assignment Details Facility
Type: Home Health. Assignment focus: Travel Physical Therapist
covering Opp, AL service area and nearby counties including
Covington, Lower Crenshaw, and West Coffee counties. Schedule: 4
days per week, full-time. (8 week assignment.) Clinical
responsibilities: patient evaluations, development and delivery of
PT plan of care, progress notes per policy, discharge summaries at
least every 60 days, coordination with physician and
interdisciplinary team. Documentation: CMS-compliant evaluation and
regular progress notes, reports to nurse supervisor within 24 hours
of evaluation. Travel requirement: clinician must visit patients
across the agency service area and nearby agencies as needed.
Requires dependable vehicle and driving between patient homes.
