Associate Clinical Director will oversee and mentor primary care
providers (PCPs) at their designated center. This role is
responsible for enhancing the execution of our core model, which
includes improving clinical quality, efficiency, outcomes, and
satisfaction for both clinicians and patients. Alongside overseeing
clinical outcomes at their center, the individual will also
dedicate part of their time to direct patient care as a PCP and/or
other clinical responsibilities, depending on the number of direct
reports. The remaining time will focus on leadership duties, such
as managing PCP performance, fostering engagement, and cultivating
a strong synergy between clinical operations and culture. The
distribution of time will vary based on factors like PCP capacity,
market demands, center size, patient membership, and guidance from
the Market Clinical Director. Key Responsibilities Provide
independent care for patients with both acute and chronic
conditions, particularly in older adults. Take full responsibility
for patient care and outcomes, seeking specialist consultations
when necessary while remaining involved in the patient's detailed
care. Collaborate with hospitalists for any of their patients
admitted to the hospital, regardless of the hospitalist's
affiliation. Conduct assessments, diagnoses, treatments,
management, education, health promotion, care coordination, and
documentation for patients with acute and complex chronic health
issues. Lead a care team that includes a care promoter (medical
assistant), care facilitator, and care coordinator for patients who
can visit the office. For patients unable to attend the
office—whether in hospitals, skilled nursing facilities, long-term
care, or homebound—coordinate with the transitional care team and
other available resources, including case managers and acute care
physicians. Facilitate the Super Huddle (SH) and Transforming Care
Meeting (TCM) weekly, and support the Center Manager or General
Manager in clinical leadership roles when necessary, following
guidance from the Market Chief Medical Officer. Step in as needed
for clinical leadership responsibilities, including monitoring
daily center census as part of joint accountability for outcomes.
Actively participate in the management of the center and provide
coverage for other providers as needed. Each Clinical Director is
also expected to assist in patient recruitment for the center and
in interviewing potential new providers. Mentor and coach PCPs in
their assigned center to achieve exceptional clinical outcomes,
including participating in daily huddles with other PCPs. Conduct
leadership rounds with PCPs, reducing the involvement of the market
clinical leader. Collaborate with the Center Operations Director
and Market General Manager to enhance the financial performance of
the center and increase membership. Perform additional duties as
assigned and modified at the discretion of management. Compensation
